This appeal arises pursuant to the Texas Workers’ Compensation Act, TEX. LAB. CODE ANN. § 401.001 et seq. (1989 Act). A contested case hearing was held on January 4, 2002. The hearing officer determined that the appellant (claimant) did not sustain a compensable occupational disease injury; that the date of the alleged injury was ____________; that the respondent (self-insured) is relieved from liability because of the claimant’s failure to timely notify the employer of an injury; and that the claimant did not have disability. On appeal, the claimant expresses disagreement with the compensability, disability, and timely notice determinations. The self-insured urges affirmance.

DECISION

The decision and order of the hearing are affirmed.

The true corporate name of the self-insured is K MART CORPORATION and the name and address of its registered agent for service of process is
